Terms and Conditions

To be eligible to win this contest, you must fulfill the required terms below. Some conditions may apply.

• Eligibility: The Vols Fan LASIK Giveaway is open to individuals who currently wear glasses or contact lenses. Participants must be at least 18 years of age at the time of entry.
• How to Enter: To enter the giveaway, participants must complete the registration form provided on TN Lasik website and agree to these terms and conditions. Entries must be submitted by October 25, 2024.
• Winner Selection: The winner will be selected at random from all eligible entries on October 29, 2024. The winner will be announced on the same day through social media and our website.
• Prize Details: The prize consists of one (1) free LASIK surgery at Tennessee Lasik for the winner only. The winner must be deemed a suitable candidate for LASIK surgery by our qualified medical staff, based on a pre-operative evaluation.
• Non-Transferable: The prize is non-transferable and cannot be redeemed for cash or any other services. The winner must personally undergo the LASIK procedure and cannot transfer their eligibility to another individual.
• Medical Evaluation: The winner will be required to schedule a consultation with our staff to determine candidacy for LASIK surgery. If the winner is not deemed a suitable candidate, the prize will be forfeited, and an alternate winner may be selected.
• General Conditions: By participating in this Giveaway, entrants agree to these Terms and Conditions and the decisions of Tennessee Lasik, which are final and binding. Tennessee Lasik reserves the right to disqualify any entry that violates these terms or is deemed inappropriate. Additional conditions may apply.

Payment Options

The cost of your surgery will depend on the options you chose for your procedure. Check with our Surgical Coordinator to discuss your options. If you are seriously considering refractive surgery or other vision correction procedures and want to make your treatment as affordable as possible, we have provided a variety of payment options for your convenience:

• Credit Card – We accept full and/or partial payments with these major credit providers: Visa, MasterCard, American Express and Discover
• Check – For your convenience, we accept checking account payments. Please note that a $100 returned check fee will be processed on all returned checks.
• Cash & Money Orders – We also accept payments in cash or with money orders. A receipt of payment will be generated for your records.
• Flex Spending Account – You should also find out if your employer's flexible spending account can be used to pay for LASIK surgery. These tax-advantaged accounts are funded through your tax-free payroll deductions and help pay for health expenses not covered by your health plan.

Financing Available

If you are unable to afford out-of-pocket surgery costs or your insurance does not cover a procedure, we offer interest-free financing for select patients who qualify.
